1/26/2024 EnchantaVaughn Approved Behavior: Upon arrival, Josiah was observed settling into the home after a long school day. Josiah was greeted by mental health professional. Josiah welcomed QMHP-C into his room today instead of sitting in the living room. Josiah was in a calm mood, as evidenced by his quiet and relaxed expressions. Josiah explained that he wanted to have session without mom present so that she could spend that time taking care of important duties around the house. Intervention: QMHP-C reviewed previous day activities and reminded Josiah of what he has to do in order to achieve his goals and objectives again today. QMHP-C reinforced Josiahâs positive behaviors by modeling positive behaviors. QMHP-C modeled proper positive behavioral expectations for Josiah. QMHP-C offered simple praises to Josiah for exhibiting the appropriate behaviors during the session. QMHP-C utilized cognitive skills to give Josiah a chance to openly and honestly discuss his feelings about his current state. QMHP-C utilized active listening skills and was empathetic with Josiah as he vented about his day. QMHP-C reiterated the importance of positive thinking and exercising his coping skills. QMHP-C supported Josiah involvement in in-home counseling. QMHP-C provided feedback to Josiah in regard to questions asked. QMHP-C explored positive behavior options with Josiah to support his willingness to make good decisions and have positive behaviors in school, at home, and in the community. QMHP-C continued to establish a positive supportive relationship with Josiah. QMHP-C will review and give feedback regarding ADHD and its causes and effect to well-being at home, as well as, his school performance. Response: Josiah was open to session and communicated openly with counselor. Josiah verbalized that heâs able to focus better when it is quieter and more peaceful. Josiah stated being in his room made him feel more comfortable and accepted. Josiah presents an alert and friendly demeanor. Josiah displayed a normal affect. Josiah was accepting of feedback provided by QMHP-C. Josiah displayed excellent cooperation and had no issues with expressing his feelings and emotions when faced with a sensitive topic. Josiah was able to show empathy toward mom by understanding the responsibilities that come with taking care of the home and having nice things. Josiah is observed being attentive and listening out for mom in case she needed his help. Josiah modeled good behavior and was appreciative of QMHP-C support and encouragement. Josiah talked a lot today but seemed to be gaining clarity about his situation. Josiah expresses his needs as it pertains to meeting his objectives and still working on other goals in his life that have nothing to do with school or home. Josiah was happy that he could confide in QMHP-C and not feel judged. Josiah recognized positive behavior methods that would be useful in school, home, and community as a way to develop skills needed to stabilize symptoms associated with ADHD. Josiah continues to build a healthy relationship with his counselor.
